• Membership Renewal This form is used to renew Club membership (note: it is not for use when joining for the first time).
• Instructor Renewal This form is used by Instructors to renew their Club membership.
• Notification of Title This form is used to notify the Club of any Titles achieved throughout the year - this information is needed for the presentation ceremony at the end of each year. If you want recognition for any Titles, then this form must be filled in.

These forms need to be printed and filled in - they cannot be emailed or submitted electronically. After completing a form, it can either be mailed to the Club at PO Box 4048, Kingston, ACT 2604 or delivered in person to the Club office (office hours are 10:00am to 1:00pm weekdays and during obedience class training times).
