• New Membership Application To join the Club, download this application form and information sheet.
• Membership Renewal This form is used to renew Club membership (note: it is not for use when joining for the first time).
• Instructor Renewal This form is used by Instructors to renew their Club membership.
• Notification of Title This form is used to notify the Club of any Titles achieved throughout the year - this information is needed for the presentation ceremony at the end of each year. If you want recognition for any Titles, then this form must be filled in.

After completing your Application or Renewal form, you can scan and email it to contact@actcdc.org.au if paying by direct credit or you can post it to the Club at PO Box 4048, Kingston, ACT 2604 or deliver it in person to the Club during office hours or during obedience class training times.
